I'm not so sure that any rules are not being followed - rather, I would bet that the problem is with the image hosts. With some hosts, if you mark a post as having adult content or if you do a mass upload but there is no option to mark the post as adult or not adult the host adds the porn banners, popups and live chat windows, etc.
These hosts typically have the same mantra: if their traffic grows past a certain point, the only way for them to turn a profit is via porn. We are expected to accept the idea that this is the price we have to pay for using their free hosting services. So far as I know, the only effective way to protest is to use a host that does not automatically add the porn crap. At the very least, use a host - and/or a bulk uploader - that provides the option to choose adult content or safe for work.
This might not explain some of the problems mentioned here, but I think that it would explain a lot of them.
